On our second popup, we had a hitch ball that was polymer, no grease required. It took a bit of adjustment when we went to the TT and had not only the hitch ball, but two smaller ones for the sway bar.
If the sway bar ones aren't lubed they groan, a lot. On my long sessions of being hitched up (NM to OH and reverse), they will groan on the last day or so, more so if I've been driving in the rain.
The main hitch ball shows wear from not having a heavy coat of grease, probably also those same long trips, when I stay hitched.

Never. I have a triple-ball (which isn't as unique as you'd think ;)) that I rotate for the different trailers. My pup doesn't have a WDH or sway bars which would probably require more rigorous maintenance.

Replacement hitch balls are cheap and easy to replace. Some have gotten rusty so we have considered a stainless one. We're about to receive a new Tacoma so maybe get a SS one for it?? Accessories, accessories, accessories....

Never on any hitch at work or at home. Now if I pulled a trailer every day I might.

As a newbie, I hadn't even thought about it. Our first tow (of anything) was our new camper - 1800 miles. There is obvious wear on the chrome of both the hitch and anti-sway bar balls. I put of thin coat of white lithium on them, and we'll see how the next long trip goes this spring.

I use extreme pressure bearing grease on the ball and in the cup. But I use anti-seize on the WD bar pivots.

It will work just fine without grease, but will wear faster and might squeak.

I grease ball and cup and about every 4-5 hitching, I wipe it out to clean it out from road dirt and re-grease it. I use wheel bearing grease that I carry in the camper.
